I am pretty sure its a slide to the fifth fret and it sounds like it is but am not sure where to slide from.....????

Hi LeeB.
Bar 5: pull off from the second fret to the open string. You can hear it and see it in the movie, and hear it in the tab.
Bar 15: Yes, slide up to the 5th fret, the movie shows that it's from the 3rd fret.
Are you not able to see the movies?
