The image below should help you understand the different types of slides. Your use of scoop and falloff was interesting as they are not common among guitar players. From your description of tie-offs above the slides, I believe they are probably legato slides where only the first note of the slide is picked. I am assuming the curved line above the slide is present as shown in the legato slides in the image below. In the image, the slide lines are very light, but if you look closely, they are there.

The beginnong scoop to fret 5 means to pick the note 2-3 or indeterminate number of frets below the shown fret, then quickly slide to the shown fret. The falloff from fret 10, means pick the note at fret 10, then slide down 2-3 or any number of frets below before releasing the pressure on the string.
