They can be used for any of those. With most things played on a guitar, there isnt really a specific crictera on what you can do. Some guitarists write the licks down, and play them the same over and over again, and some improvise. Depends on the preference of Guitarist/Band.
Now as for when to use them, The writer(s) of the songs for a band tends to put them in where he/she sees fit
For #3, some bands use them as fillers, some use them as main riffs, some just mix it up. I myself do it on what I hear in my head's sound when i'm writing, so depending on the sound, i write to match it.
Again, it's down to songwriter preference.
For 4: Some people use the Pentronic (Sp?) Scale, some just expiriment, or in my case, follow the notes in my head. Songwriter prefrences again.
